You can also hand in your application form to any of your class instructors along with a cheque or cash for the appropriate amount.

APPLICATION FORM

For applicants between 7-18 years old:
I give my consent for my son/daughter/ward to be a member of Persatuan Yoshinkan Aikido Kuala Lumpur & Selangor and to participate in its activities.

EXAMPLE
If you are above 18 and wish to join, then you have to pay a one-time registration fee of RM20. If you choose to pay your subscription fee every month, then you will need to pay in total RM20 + RM5 = RM25 in the first month and then RM5 every month after that.

But if you choose to pay your annual subscription at one, then you have to pay RM20 + RM60 = RM80 in the first year and then RM60 every year after that.
